• Does the employee have adequate tools, equipment and support?
  • Has his performance been objectively assessed?
  • Have his shortcomings been pointed out to him/her (when and how many times?)
  • Is there a domestic situation that may be affecting his work (substance abuse or family     problems?)
  • Has he/she been made aware of consequences of failing to achieve the required work standard?
  • Has he/she had enough time to improve/achieve the work standard?
  • Are there extenuating, mitigating or aggravating factors?
  • Has counselling been recorded?
  • Is there an alternative to dismissal?
